Thelma Gheen Smith Kozak of Vienna, VA, died on Wednesday, December 27, 2006 at Fairfax Nursing Center. Wife of the late George Smith and Harry Kozak; mother of Jeannie Burton, Greg Kozak, Pamela Meade and Barry Kozak. Also survived by 12 grandchildren and one great-grandchild. Graveside services will be held on Saturday, December 30 at 2 p.m. at Fairfax Memorial Park. Please meet at the cemetery office at 1:45 p.m. The family suggest that memorial contributions be made to Fairfax Nursing Center, 2nd Floor Employment Fund, 10701 Main St., Fairfax, VA 22031.
